Job Description
- Facilitate and manage the referral process for patients between primary and secondary healthcare facilities.
- Serve as a liaison between healthcare providers to ensure smooth communication and patient transitions.
- Verify that all referral documentation is completed, accurate, and meets project guidelines.
- Monitor the status of referred patients to ensure they receive timely care and follow-up appointments.
- Collect and maintain comprehensive data on all referrals, including patient demographics, treatment timelines, and outcomes.
- Prepare and submit regular reports on referral activities, trends, and project performance to management.
- Identify and address challenges or delays in the referral process, proposing solutions where necessary.
- Ensure adherence to data protection policies and maintain confidentiality of patient information.
- Collaborate with healthcare teams to improve referral pathways and address gaps in service delivery.
- Provide support and training to staff involved in the referral process, ensuring consistent implementation of best practices.

Job Requirements
- Bachelor's degree in any relevant field.
