Job Description

  • Facilitate and manage the referral process for patients between primary and secondary healthcare facilities.
  • Serve as a liaison between healthcare providers to ensure smooth communication and patient transitions.
  • Verify that all referral documentation is completed, accurate, and meets project guidelines.
  • Monitor the status of referred patients to ensure they receive timely care and follow-up appointments.
  • Collect and maintain comprehensive data on all referrals, including patient demographics, treatment timelines, and outcomes.
  • Prepare and submit regular reports on referral activities, trends, and project performance to management.
  • Identify and address challenges or delays in the referral process, proposing solutions where necessary.
  • Ensure adherence to data protection policies and maintain confidentiality of patient information.
  • Collaborate with healthcare teams to improve referral pathways and address gaps in service delivery.
  • Provide support and training to staff involved in the referral process, ensuring consistent implementation of best practices.

Job Requirements

  • Bachelor's degree in any relevant field.
  • Strong organizational and multitasking skills with attention to detail.
  • Experience in healthcare coordination, patient referrals, or project management is preferred.
