Job Description

Primary Role:

  • Serve as the key point of contact for managing patient referrals within the SHC project, including those referred from the PHC project. Ensure accurate documentation and processing of cases while preparing them for discussions with the ECC.

Responsibilities:

  1. Referral Case Management:
    • Receive and review referral cases from governorate doctors, elective doctors, and the PHC project.
    • Verify medical necessity and ensure all required documentation is complete for each referral case.
  2. PHC Referral Integration:
    • Handle patient referrals originating from the PHC project, ensuring smooth integration into the SHC project workflow.
    • Collaborate with the PHC project team to address any documentation or patient history gaps before submission.
  3. ECC Preparation:
    • Organize and prepare patient cases for presentation to the ECC, ensuring data accuracy and compliance with project protocols.
    • Address follow-ups or additional information requests from the ECC regarding referred cases.
  4. Documentation & Reporting:
    • Maintain a comprehensive database of all referral cases, tracking their progress, outcomes, and any challenges.
    • Provide regular updates to the Project Section Head regarding referral activities, focusing on PHC cases and outcomes.
  5. Patient Advocacy:
    • Ensure referred patients, including those from the PHC project, are fully informed about the next steps in their care.
    • Address patient concerns or barriers related to their referral process and escalate issues when necessary.
  6. Quality Assurance:
    • Monitor the referral process for any inefficiencies, particularly in handling PHC referrals, and recommend improvements to the Project Section Head

Job Requirements

Education & Experience:

  • Doctor of Medicine (MBBCh) degree or equivalent from a recognized institution.
  • Minimum 2 years of clinical experience, preferably in referral case management, emergency medicine, internal medicine, or family medicine.
  • Experience in healthcare coordination, patient referrals, or case management in a hospital or humanitarian setting is highly desirable.
  • Prior experience working with refugees, asylum seekers, or vulnerable populations is an advantage.
  • Familiarity with UNHCR healthcare policies and referral mechanisms is a strong plus.

Skills & Competencies:

  • Medical & Referral Expertise: Strong understanding of medical case referral processes, including documentation and patient assessment.
  • Case Review & Coordination: Ability to assess medical necessity, review patient files, and ensure completeness of documentation.
  • Data Management & Reporting: Experience in maintaining patient databases, tracking case progress, and compiling reports for project leadership.
  • Communication & Collaboration: Strong interpersonal skills to liaise with PHC teams, governorate doctors, elective doctors, and external healthcare providers.
  • Problem-Solving & Decision-Making: Ability to identify gaps in referrals, resolve issues, and facilitate smooth patient transitions.
  • Patient Advocacy: Commitment to ensuring that referred patients receive appropriate care and understand their treatment pathways.

Technical Proficiency:

  • Proficient in Microsoft Office (Excel, Word, PowerPoint) for tracking referrals and reporting.
  • Experience using medical records systems or referral tracking software is a plus.

Languages:

  • Proficiency in English and Arabic (both written and spoken) is required.
  • Knowledge of French, Tigrinya, Somali, or other refugee languages is an advantage.
