Job Description

Key Responsibilities:

  • Coordinate with doctors, nurses, and relevant departments to ensure integrated patient care.
  • Identify and analyze deficiencies in care plans and correct them using available resources.
  • Conduct regular rounds in nursing units to review new admissions and update on ongoing cases.
  • Facilitate early discharge for patients who have completed their care plans.
  • Review requested investigations, procedures, and consultations, and ensure prompt service delivery.
  • Ensure accurate documentation and adherence to care plans.
  • Create and update a daily list of "high-priority patients" for close monitoring.
  • Monitor complex cases requiring special attention (e.g. abnormal lab results, extended length of stay, level of care changes, ICU/wings mismatch, re-admissions, transfusions).
  • Collect and analyze data to support evidence-based care decisions.
  • Submit a daily case management report to the Medical Director.
  • Prepare a comprehensive monthly report including: occupancy rates, bed turnover, patient distribution, source of admission, average length of stay, mortality rate, surgical classification, service utilization, and recommendations for improvement.

Job Requirements

Education:

  • Bachelor’s degree in Pharmacy.
  • 0–2 years of experience in Case Management or a similar field.
  • Awareness of Clinical Practice Guidelines (CPGs).
  • Preferred: Diploma or Master's in Hospital Management, Total Quality Management, or MBA.
