• Job Details:1. Plan, organize and conduct rehabilitation assessment, evaluation and intervention for clients on caseload. 2. Teach clients independent living skills and community integration skills. 3. Provide supportive counseling to severely emotionally distressed clients on caseload. 4. Review and coordinate the collection for relevant information on referred clients' background, social history and past treatment interventions in order to develop treatment plans for clients. 5. Develop and implement individualized rehabilitation plans for assigned clients to meet individual needs, interest and abilities. 6. Evaluate, monitor and review individualized rehabilitation plans and assess individual progress in treatment and make recommendations for modification, changes or termination of treatment. 7. Maintain records as required by established laws, established policy, and administrative regulations including COMAR regulations. 8. Attend all discharge, termination, admission and clinical supervision as scheduled to promote client services or professional growth. 9. May act as a consultant to outside providers, school personnel and medical staff on the individual needs of clients on caseload. 10. Consult with medical staff to coordinate services to clients.
