• Job Details:-Make contact with local community leaders to Provide full case management support to community members -Development and implementation of detailed care plans -Update records regularly and preparing reports on caseload -Following-up and monitoring on individual cases -Referral parents, youth and children to participate in center activities or awareness session to satisfy their needs and increase awareness. -Provide support to beneficiaries according to service map -Supporting the project team in developing the project activities and the monthly plan -Supporting the program teams as necessary
