• Job Details:- Reviews, analyzes, and codes medical record documentation to include, but not limited to, medical, diagnostic and procedural information for the correct ICD-10-CM and/or CPT and/or HCPCS codes to the greatest specificity - Ensures that inpatient and outpatient records are coded, abstracted and entered into an internal and external computer system in an accurate and timely manner - Perform inpatient tasks consisting of performing a comprehensive review of documentation within the health record or assigning ICD-10 codes for diagnosis, complications, co-morbid conditions, surgery and procedures - Perform outpatient tasks consisting of performing a comprehensive review of documentation within the health record to accurately assign ICD-10 codes for diagnosis and complications, and CPT codes for surgeries, procedures, and evaluation, and management services - Maintain a 95% quality audit accuracy rate
