3.1. Taking patient medication history
3.2. Prioritize level of care based on critical need
3.3. Participate in interdisciplinary patient care rounds and communicate and follow up with clinical team.
3.4. Prevent, detect and resolve medication related problems to make evidence – based patient centered medication therapy recommendation.
3.5. Verify patient medication orders for appropriateness and accuracy
3.6. Document critical intervention
3.7. Provide drug information for patients and all health care team
3.8. Provide patient education for patients about their medications administration and precautions.
3.9. Provide discharge medication teaching upon request
3.10. Report medication errors and ADRs
3.11. Participate collegially in the development of new programs, services and practices, educational activities of the pharmacy department and mentor clinical pharmacy interns.
3.12. Establish regular meetings with chief pharmacist.
3.13. Identify potential technical or patient-care issues and work to make recommendations to avoid issues. 3.14. Monitor newly added medications upon request.
3.15. Participates and provides consultation to hospital committees, such as Pharmacy and Therapeutics (P&T) and clinical team committee when pharmacotherapy issues are discussed.
3.16. Participates in clinical polices development, disease management, and drug use program as identified and under the membership of the chief pharmacist and collegially with MMU plan.
