Job Description
- 3.1. Taking patient medication history
- 3.2. Prioritize level of care based on critical need
- 3.3. Participate in interdisciplinary patient care rounds and communicate and follow up with clinical team.
- 3.4. Prevent, detect and resolve medication related problems to make evidence – based patient centered medication therapy recommendation.
- 3.5. Verify patient medication orders for appropriateness and accuracy
- 3.6. Document critical intervention
- 3.7. Provide drug information for patients and all health care team
- 3.8. Provide patient education for patients about their medications administration and precautions.
- 3.9. Provide discharge medication teaching upon request
- 3.10. Report medication errors and ADRs
- 3.11. Participate collegially in the development of new programs, services and practices, educational activities of the pharmacy department and mentor clinical pharmacy interns.
- 3.12. Establish regular meetings with chief pharmacist.
- 3.13. Identify potential technical or patient-care issues and work to make recommendations to avoid issues. 3.14. Monitor newly added medications upon request.
- 3.15. Participates and provides consultation to hospital committees, such as Pharmacy and Therapeutics (P&T) and clinical team committee when pharmacotherapy issues are discussed.
- 3.16. Participates in clinical polices development, disease management, and drug use program as identified and under the membership of the chief pharmacist and collegially with MMU plan.
Job Requirements
- Education: Bachelor degree in pharmaceutical science.
- Experience: 1-2 experiences in hospital setting
