Job Description

Summary

Develops plans, organizes, and coordinates the Hospital’s overall medication safety programs. Provides leadership for medication safety assessments, coordinates the medication safety activities, educates others on the system-based cause for medical error, consults with concerned areas, and communicates literature-based programs that ensure medication safety strategies within Hospital.



Essential Responsibilities and Duties
  1.  Identifies andimplements best practices for medication safety that will support the reductionof medication errors and other factors that contribute to unintended adversemedication outcomes.
  2. Analyses the Hospital’scurrent medication safety practices that contribute to medication errors andtake proactive steps for prevention.
  3. Oversees the management and use of medicationerror information. Reviews internal error report and utilize information fromexternal reporting programs (e.g. ISMP Medication Safety Alert and ECRI HealthDevice Alerts).
  4. Manages medication-errorreporting, reviews investigations to determine the root cause, and data entryinto the database. Analyses data determine process improvement trends anddevelops the required measures to eliminate the likelihood of occurrence and recurrenceof errors.
  5. Participates in the designand development of competency assessments for evaluating pertinent healthcareproviders' knowledge and ability to perform medication safety tasks.
  6. Standardizes relateddefinition, classification, and reporting of medication error in the Hospital.Leads the development and review of all medication use, medication safety, andmedication-error reporting policies and procedures.
  7. Educates pharmacy staff andother clinicians to promote safe medication practices. Develops and implementsthe most up-to-date educational programs to improve patient medication safetyand regulatory compliance.
  8. Designs and implementseducational presentations that facilitate the understanding and implementationthe medication safety standards within the Hospital, including dangerousabbreviations and illegible orders.
  9. Supports and encourageserror reporting throughout the Hospital through a culture error reportingsystem and development of a recognition program to improve medication safety.
  10. Develops and implementsmedication control and safety standards in Hospital related Pharmacy patientcare, routinely monitors compliance, and conducts a Hospital-wide assessment ofpatient safety to create a baseline measurement of the safety culture and theprevalence of risk or error.
Education

Master’s or Bachelor’s Degreein Pharmacy from an accredited college is required.



Experience Required

Eight (8) years of relatedexperience with Master’s, or ten (10) years of related experience with Bachelor’sDegree is required.



Other Requirements(Certificates)

Current Registered fromcountry of origin and Saudi Commission for Health Specialties Licensure isrequired.



Certified Professional Healthcare Quality (CPHQ) ispreferred.
