Job Description

POSITION TITLE: Medical Records Porter

Responsible for monitoring of requests, delivery and retrieval of health information from clinical and other specified areas, using the electronic request and delivery tracker, assisting with sorting of loose information received (reviewing and shredding content not part of the legal health record and not belonging to health records), assisting with pulling and filing of charts and records.


DUTIES AND RESPONSIBILITIES:


  • Maintains record availability by processing delivery and retrieval using chart mark-off procedures; facilitating chart location activities.

  • Retrieves medical records by following chart-out procedures; documenting reasons charts cannot be retrieved for statistical and follow-up purposes. 

  • Delivers charts to assigned areas of the hospital by following established routing procedures.

  • Keeps health care providers informed by communicating availability or unavailability of the record.

  • Collection of Emergency Department records as per the schedule.

  • Delivery of health records in the approved delivery system while ensuring safety confidentiality and security.

  • Sort loose information in the appropriate format and sorter.

  • Filing of charts in the appropriate storage /filing areas.

  • Interdepartmental shredding as per identified documents.

  • Match all loose reports using at least two patient identifiers.

  • Secure work area.

  • Collection, as per the schedule at pre-defined periods.

  • Maintains confidentiality of all data and information at all times. 

  • Delivery of requested records as per the need and the request.

  • Prints pull list and tracer cards of booked clinic appointments and prints clinic add-on list and prepares patient records accordingly. 

  • Appropriate usage of chart tracking module ensuring that patient record locations are continuously updated.

QUALIFICATIONS:


  • High School Diploma preferred.

PROFESSIONAL EXPERIENCE:


  • Experience a recommendation (Medical Records / Health Informatics). 
  • Knowledgeable about security and confidentiality.
  • Ability and comprehension in written and spoken English.
