Job Description
- Find and investigate all medication errors reported through the system.
- Implement corrective actions for the reported medication error.
- Audit the use of high alert medications and concentrated electrolytes in all areas of the hospital.
- Providing on the job training on medication handling as a result of the medication error findings to ensure medication safety.
- Perform audits on medication prescribing, dispensing and administration to ensure that the correct medication is administered in the correct dose to the correct patient.
- Perform other related duties as assigned.
- Direct manager will be the Medical Director
Job Requirements
- Experience as clinical pharmacist
- Experience in Medication safety
- BA in Pharmacy is Must
- Advanced training in Medication Safety will be nice
- Working hours 8 hrs/ day - 6 days /week & 1 day off
