Job Description
- Works in a team based environment to help achieve maximum benefit from our services through care integration.
- Handles inbound calls from patients directly or calls transferred from case management nurse/patient advocate and perform periodic outbound calls to ensure that patients are directed to and receive care in appropriate venue in a timely manner.
- Takes and records detailed history in electronic record format.
- Provide remote treatment when/if condition could be managed in house while following MD24-7 policies and procedures.
- May offer in-home assessment to patients with identified barriers to appropriate healthcare facilities.
- Remotely monitor enrolled patients through our telemonitoring software.
- Enrolls patients in our Chronic Care program and perform complete assessments for identified patients who may benefit from this program.
Job Requirements
- Basic medical degree (MBBS or equivalent.)
- At least 5 years’ clinical experience post-graduation.
- Worked previously in the capacity of General Practitioner, Family Medicine or equivalent.
- Candidate should be bilingual.
- Should be available to work 48 hours per week including some night shifts.
